Chromosomal aneuploidy is a relatively common condition in human embryos, with rates ranging from 20% to 80% depending on maternal age. Approximately 10% of clinical pregnancies are found to have triploidy or monosomy, and this rate can rise to 50% in older pregnant women. Aneuploidy can lead to implantation failure or be a major cause of recurrent miscarriage.
The ASEM test evaluates the chromosomal genetic status of embryos through the embryo culture medium.
Preimplantation Genetic Testing for Aneuploidy (PGT-A) is currently recommended for couples undergoing IVF to select embryos without chromosomal aneuploidy for transfer.
PGT-A has been shown to increase successful embryo transfer rates, shorten the time to conception through euploid embryo transfer, and reduce the number of transfers, miscarriage rates, and overall costs, especially in older women. To perform PGT-A, embryo biopsy is required to obtain the necessary material for testing.
In contrast, the ASEM test (Analysis of Spent Embryo Medium) assesses the chromosomal genetic status of embryos through the embryo culture medium without requiring an embryo biopsy. The ASEM test is based on the presence of cell-free DNA (cfDNA) in the culture medium. Studies have shown that blastocyst culture medium can be used to evaluate the genetic status of embryos.
Advantages of the ASEM Test
Because the ASEM test is performed on the embryo culture medium, it is non-invasive and safe.
Unlike traditional methods that require embryo biopsy, the ASEM test offers several notable advantages:
- Reduced Costs: It eliminates the need for equipment investment for embryo biopsy and the requirement for skilled embryologists to perform the biopsy.
- Non-Invasive: The ASEM test does not disturb the embryo, which is particularly beneficial for poorly developing embryos where biopsy could be detrimental. It allows for the assessment of chromosomal aneuploidy before embryo transfer without invasive procedures.
- Useful in Certain Situations: In cases where embryo biopsy for PGT-A cannot be performed for various reasons, the ASEM test provides valuable genetic information about the embryo.
The Role of the ASEM Test in Assisted Reproduction
The ASEM test represents a breakthrough in genetic diagnosis and is a valuable tool in the field of assisted reproduction. When combined with morphological evaluation, it helps select embryos with the best potential for transfer in cases where embryo biopsy for PGT-A is not feasible. The goal is to increase the success rates of embryo transfers and reduce miscarriage rates for infertile couples.
ASEM Testing at GENTIS
GENTIS is the first institution in Vietnam to implement and offer the ASEM test. Mr. Nguyen Quang Vinh, MSc, Director of the GENTIS Testing Center, stated: "GENTIS continues to research and evaluate the effectiveness of the ASEM test to improve the quality of IVF/ICSI cycles, with the aim of enhancing successful embryo transfer rates."
To assess the clinical effectiveness of the ASEM test, GENTIS has collaborated with several hospitals and assisted reproduction centers nationwide. Notably, GENTIS and Hanh Phuc International Hospital conducted a study titled "Evaluation of the Effectiveness of Genetic Analysis of Embryo Culture Medium."
The study compared clinical outcomes between two groups: one that selected embryos for transfer based solely on morphological evaluation (control group) and another that used both morphological evaluation and ASEM test results (study group). The study found live birth rates of 38.6% in the control group and 41.1% in the study group. Additionally, the miscarriage rate in the study group was 0% (no miscarriages were recorded) compared to 12.9% in the control group.
These preliminary findings suggest that the ASEM test has the potential to increase live birth rates and significantly reduce miscarriage rates. This is crucial in preventing the physical and emotional trauma associated with miscarriage.
The initial successes of the ASEM test further solidified GENTIS’s leading position in the field of genetic analysis in Vietnam. GENTIS hopes that the ASEM test will become a routine clinical practice in hospitals and IVF centers worldwide, bringing joy and happiness to couples of reproductive age who need medical assistance to conceive and give birth to healthy babies.

To foster cooperation and advance research on a range of new services, GENTIS organized the seminar at Ngoc Bich General Hospital in Nam Dinh. The event featured the participation of Dr. Nguyen Van Hoa, Chief Medical Officer; Dr. Nguyen Van Hung, Deputy Director of the hospital; Dr. Vu Duy Trong, Deputy Head of Surgery; Mr. Tran Minh Chau, Head of the Planning Department; along with over 20 other doctors and medical staff from the hospital. GENTIS was represented by Ms. Le Thi Hong Nhung from the product management department.
During the seminar, presenter Le Thi Hong Nhung provided a detailed report on various tests currently being researched and developed by GENTIS. These tests have significant implications in the fields of reproductive health, obstetrics, and pediatrics, and include:

The seminar began with a comprehensive presentation by Ms. Hong Nhung on GENTIS's GenEva NIPT, a non-invasive prenatal screening test. This test can detect chromosomal abnormalities in fetuses, such as Down syndrome, Edwards syndrome, Patau syndrome, and microdeletions/microduplications.
The test is performed through maternal blood sampling, ensuring safety for both the mother and the fetus. The testing process is optimized, fast, and provides results with an accuracy rate of over 99%.
Additionally, the newborn screening tests offered by GENTIS garnered significant interest from the attending medical staff. Newborn screening is regarded as one of the most effective methods for early detection of abnormalities in infants, allowing for timely intervention and preventing severe complications and damage.
Moreover, newborn screening increases survival rates, reduces the impact of diseases, extends patient lifespans, and helps save on healthcare costs.
As part of the seminar, Ms. Hong Nhung also presented on tests for Thalassemia, Thrombophilia, Antiphospholipid Syndrome, HPV - EPREP, and more.

In his role as chair of the CME3 session, Prof. Dr. Nguyen Dinh Tao (President of the Assisted Reproduction Association) coordinated the program and provided clear insights into the importance of genetic testing in assisted reproduction.
The training session began with a presentation by Assoc. Prof. Dr. Le Hoang (Director of the Assisted Reproductive Technology Center, Tam Anh General Hospital) on the topic "The Role of Genetics in Assisted Reproduction," which attracted the attention of numerous attending doctors. According to Assoc. Prof. Le Hoang, the integration of genetics and assisted reproduction creates new prospects for enhancing treatment effectiveness for infertile patients. Moreover, it is crucial to focus on developing experts in the field of genetics to support clinical doctors. Ordering, counseling, and interpreting genetic test results will be a challenge for clinical doctors who have not yet had much exposure to this field.
The next presented topic, "Mosaic Embryo Transfer: International Recommendations and Clinical Practice in Vietnam," shared by Assoc. Prof. Dr. Ho Sy Hung (Deputy Director of the National Assisted Reproductive Technology Center, Obstetrics and Gynecology Hospital), garnered significant interest from assisted reproduction doctors. In this report, GENTIS was honored to provide survey data from the National Assisted Reproductive Technology Center in 2023, with 513 embryos examined, revealing a mosaic embryo rate of 30.6%. The associate professor also highlighted points to consider when interpreting embryo test results in cases of mosaicism, and the order of priority for transferring mosaic embryos according to international standards and clinical practice in Vietnam.
Beyond female treatment, the topic of male infertility, "Y Chromosome Abnormalities Causing Infertility," was also an engaging presentation by Assoc. Prof. Dr. Trinh The Son (Director of the Military Clinical Embryo Institute). The associate professor identified Y chromosome abnormalities such as translocations, microdeletions, and Y chromosome numerical abnormalities causing various male infertility conditions like 46,XX; AZF a,b,c deletions; Klinefelter; 47,XYY… From there, Assoc. Prof. Son shared clinical experiences of successfully treated male infertility cases and numerous studies conducted worldwide.
At the end of the CME3 training session, Dr. Pham Dinh Minh (Director of GENTIS R&D Center) provided clinical doctors with an update on the topic "Applying Advances in Biotechnology to Develop Gene and Genetic Tests in Assisted Reproduction." As a pioneer in the field of genetics, GENTIS has provided a comprehensive gene ecosystem for applications from the pre-pregnancy, pregnancy, and healthy childbirth stages. Dr. Minh particularly emphasized the role of genetic doctors and genetic counseling for patients to ensure the best treatment options for infertile families.
The program concluded with lively discussions from participating delegates revolving around topics such as: screening and diagnostic testing for Thalassemia and methods to help families with the disease have healthy children; follow-up recommendations for pregnancy after mosaic embryo transfer; the potential of embryo culture environment analysis in assisted reproduction…

Speaking at the opening of the event, Mr.Do Manh Ha - General Director of GENTIS Company hopes that the Coffee Talk event series will become a reliable forum for experts, doctors, technicians to exchange, share and learn from experience in the field of assisted reproduction.
Throughout the program, speakers talked about the difficulties and challenges of clinicians, embryologists, genetic testing labs, genetic consultants when performing and using PGT tests. Preimplantation genetic testing (PGT) or embryo screening tests with three main types: screening of abnormal chromosomal quantities, screening of abnormal chromosomal structure, and screening for single-gene disease.

All three of these techniques are being routinely applied in IVF centers in Vietnam. However, there are still some problems with standardization, how to improve quality, when to consult before implementation, to designate the right group of patients to be performed, to advise on PGT test results, to consider embryo transfer,... Those are the issues discussed at this Coffee talk. Under the chairmanship of Assoc.Prof.Trinh The Son, PhD.MD. and Nguyen Khac Han Hoan, PhD.MD.,, the speakers also shared practical problems, suggestions and solutions to improve the quality of PGT testing.
The sharing session was very open and in-depth on each issue, bringing a lot of useful information, helping experts better understand PGT testing. Closing the event, Assoc. Prof. Trinh The Son, PhD.MD. shared: "I am very happy to participate in Coffee Talk No. 1 as the chairman, I hope GENTIS Genetic Counseling Center will soon launch sessions 2 and 3 with more attractive and useful topics".

European Society of Gynecology - ESG is a prestigious international organization in the field of Obstetrics and Gynecology. Since 1995, ESG conferences have brought together the world's leading experts in the field to exchange knowledge, share clinical research and professional experiences. These conferences typically occur every two years. In 2023, the 15th conference of ESG took place from November 29th to December 2nd in Amsterdam, Netherlands.
As a globally recognized organization, the scientific reports presented directly at the conference undergo a rigorous selection process. At the 2023 conference, Vietnam was honored to be the sole representative with a directly presented report from the research team of GENTIS and Hanoi Hospital of Andrology and Infertility on Preimplantation Genetic Testing for Monogenic Disorders (PGT-M).
Specialist level I Ho Van Thang, a Reproductive Medicine Specialist at Hanoi Hospital of Andrology and Infertility, along with his research team from the hospital and GENTIS, successfully submitted their research paper. After passing rigorous international peer review, they were honored to be the sole Vietnamese representative to give a live presentation at the main session on November 30th. Their presentation focused on "Initial evaluation of preimplantation genetic testing for monogenic disorders (PGT-M) in patients with single-gene genetic diseases”.
PGT-M is a genetic test that enables couples carrying genetic diseases to select embryos that are free from the specific genetic mutations causing the disease. This allows them to transfer healthy embryos to the mother's uterus, preventing the transmission of genetic disorders to the next generation. In a collaborative study conducted by GENTIS and Andrology and Infertility Hospital of Hanoi from January 2020 to May 2023 involving over 20 couples (excluding those with Thalassemia), PGT-M was used to screen embryos for various genetic disorders, including autosomal dominant (polycystic kidney disease), autosomal recessive (spinal muscular atrophy; Arthritis - kidney dysfunction - cholestasis; hemophagocytic lymphohistiocytosis; Atypical hemolytic-uremic syndrome; X-linked Duchenne muscular dystrophy; hemophilia A; hemophilia B; X-linked myotubular myopathy). The clinical outcomes demonstrated a pregnancy rate of 66.7% and a live birth rate of 29.2%.
The report highlighted the significant role of preimplantation genetic testing (PGT-M) in helping couples with monogenic disorders have healthy offspring. A close collaboration among clinical physicians, genetic specialists, and embryologists is essential for accurate diagnosis, effective treatment, and optimal outcomes for patients undergoing in vitro fertilization.
Specialist level I Ho Van Thang's presentation at the main plenary session garnered significant interest and discussion from representatives of healthcare organizations worldwide, particularly those with advanced reproductive medicine programs.
Participating in the 15th European Society of Gynecology Conference provided a valuable opportunity for GENTIS and Hanoi Hospital of Andrology and Infertility to connect with the global reproductive medicine community. The presentation showcased the results of years of dedicated research aimed at developing innovative technologies and improving treatment outcomes for infertile couples. This aligns with GENTIS's long-term goal of helping more couples realize their dream of parenthood.
