On the occasion of the 75th anniversary of the Vietnam Military Medical Academy (March 10, 1949 - March 10, 2024), the Academy organized a scientific conference themed "Updating Knowledge, Applying Technology, Enhancing the Quality of Protection and Healthcare for Soldiers and People" on March 4-5, 2024.

Taking place over two days, the Continuing Medical Education (CME) program and the official scientific conference attracted over 500 delegates, encompassing training topics such as:

CME1: Updating Breast Reconstruction and Reconstructive Surgery Techniques and Post-Breast Cancer Treatment Complications.

In his role as chair of the CME3 session, Prof. Dr. Nguyen Dinh Tao (President of the Assisted Reproduction Association) coordinated the program and provided clear insights into the importance of genetic testing in assisted reproduction.

The training session began with a presentation by Assoc. Prof. Dr. Le Hoang (Director of the Assisted Reproductive Technology Center, Tam Anh General Hospital) on the topic "The Role of Genetics in Assisted Reproduction," which attracted the attention of numerous attending doctors. According to Assoc. Prof. Le Hoang, the integration of genetics and assisted reproduction creates new prospects for enhancing treatment effectiveness for infertile patients. Moreover, it is crucial to focus on developing experts in the field of genetics to support clinical doctors. Ordering, counseling, and interpreting genetic test results will be a challenge for clinical doctors who have not yet had much exposure to this field.

The next presented topic, "Mosaic Embryo Transfer: International Recommendations and Clinical Practice in Vietnam," shared by Assoc. Prof. Dr. Ho Sy Hung (Deputy Director of the National Assisted Reproductive Technology Center, Obstetrics and Gynecology Hospital), garnered significant interest from assisted reproduction doctors. In this report, GENTIS was honored to provide survey data from the National Assisted Reproductive Technology Center in 2023, with 513 embryos examined, revealing a mosaic embryo rate of 30.6%. The associate professor also highlighted points to consider when interpreting embryo test results in cases of mosaicism, and the order of priority for transferring mosaic embryos according to international standards and clinical practice in Vietnam.

Beyond female treatment, the topic of male infertility, "Y Chromosome Abnormalities Causing Infertility," was also an engaging presentation by Assoc. Prof. Dr. Trinh The Son (Director of the Military Clinical Embryo Institute). The associate professor identified Y chromosome abnormalities such as translocations, microdeletions, and Y chromosome numerical abnormalities causing various male infertility conditions like 46,XX; AZF a,b,c deletions; Klinefelter; 47,XYY… From there, Assoc. Prof. Son shared clinical experiences of successfully treated male infertility cases and numerous studies conducted worldwide.

At the end of the CME3 training session, Dr. Pham Dinh Minh (Director of GENTIS R&D Center) provided clinical doctors with an update on the topic "Applying Advances in Biotechnology to Develop Gene and Genetic Tests in Assisted Reproduction." As a pioneer in the field of genetics, GENTIS has provided a comprehensive gene ecosystem for applications from the pre-pregnancy, pregnancy, and healthy childbirth stages. Dr. Minh particularly emphasized the role of genetic doctors and genetic counseling for patients to ensure the best treatment options for infertile families.

The program concluded with lively discussions from participating delegates revolving around topics such as: screening and diagnostic testing for Thalassemia and methods to help families with the disease have healthy children; follow-up recommendations for pregnancy after mosaic embryo transfer; the potential of embryo culture environment analysis in assisted reproduction…

Currently, genetic counseling in assisted reproduction is important but still a difficult topic. Therefore, there should be mutual exchange, support, agreement between clinicians, genetic specialists, embryologists and laboratory tests.

Understanding that, the Genetic Counseling Center of GENTIS held a Coffee Talk: Genetic Counseling in the field of assisted reproduction No. 1 entitled "Specialist Consultation of PGT testing”. The event was hosted by Assoc.Prof. Trinh The Son PhD.MD. - Director of the Military Institute of Clinical Embryo Histology and Nguyen Khac Han Hoan, PhD.MD. - Vice President of the Vietnam Association of Medical Genetics - Head of the Department of Medical Genetic Testing, Tu Du Hospital.

Speaking at the opening of the event, Mr.Do Manh Ha - General Director of GENTIS Company hopes that the Coffee Talk event series will become a reliable forum for experts, doctors, technicians to exchange, share and learn from experience in the field of assisted reproduction.

Throughout the program, speakers talked about the difficulties and challenges of clinicians, embryologists, genetic testing labs, genetic consultants when performing and using PGT tests. Preimplantation genetic testing (PGT) or embryo screening tests with three main types: screening of abnormal chromosomal quantities, screening of abnormal chromosomal structure, and screening for single-gene disease.

All three of these techniques are being routinely applied in IVF centers in Vietnam. Under the chairmanship of Assoc.Prof.Trinh The Son, PhD.MD. and Nguyen Khac Han Hoan, PhD.MD.,, the speakers also shared practical problems, suggestions and solutions to improve the quality of PGT testing.

The sharing session was very open and in-depth on each issue, bringing a lot of useful information, helping experts better understand PGT testing. Closing the event, Assoc. European Society of Gynecology - ESG  is a prestigious international organization in the field of Obstetrics and Gynecology. Since 1995, ESG conferences have brought together the world's leading experts in the field to exchange knowledge, share clinical research and professional experiences. These conferences typically occur every two years. In 2023, the 15th conference  of ESG took place from November 29th to December 2nd in Amsterdam, Netherlands. 

As a globally recognized organization, the scientific reports presented directly at the conference undergo a rigorous selection process. At the 2023 conference, Vietnam was honored to be the sole representative with a directly presented report from the research team of GENTIS and Hanoi Hospital of Andrology and Infertility on Preimplantation Genetic Testing for Monogenic Disorders (PGT-M).

Specialist level I Ho Van Thang, a Reproductive Medicine Specialist at Hanoi Hospital of Andrology and Infertility, along with his research team from the hospital and GENTIS, successfully submitted their research paper. After passing rigorous international peer review, they were honored to be the sole Vietnamese representative to give a live presentation at the main session on November 30th. Their presentation focused on "Initial evaluation of preimplantation genetic testing for monogenic disorders (PGT-M) in patients with single-gene genetic diseases”.

PGT-M is a genetic test that enables couples carrying genetic diseases to select embryos that are free from the specific genetic mutations causing the disease. This allows them to transfer healthy embryos to the mother's uterus, preventing the transmission of genetic disorders to the next generation. In a collaborative study conducted by GENTIS and Andrology and Infertility Hospital of Hanoi from January 2020 to May 2023 involving over 20 couples (excluding those with Thalassemia), PGT-M was used to screen embryos for various genetic disorders, including autosomal dominant (polycystic kidney disease), autosomal recessive (spinal muscular atrophy; Arthritis - kidney dysfunction - cholestasis; hemophagocytic lymphohistiocytosis; Atypical hemolytic-uremic syndrome; X-linked Duchenne muscular dystrophy; hemophilia A; hemophilia B; X-linked myotubular myopathy). The clinical outcomes demonstrated a pregnancy rate of 66.7% and a live birth rate of 29.2%.

The report highlighted the significant role of preimplantation genetic testing (PGT-M) in helping couples with monogenic disorders have healthy offspring. A close collaboration among clinical physicians, genetic specialists, and embryologists is essential for accurate diagnosis, effective treatment, and optimal outcomes for patients undergoing in vitro fertilization.

Specialist level I Ho Van Thang's presentation at the main plenary session garnered significant interest and discussion from representatives of healthcare organizations worldwide, particularly those with advanced reproductive medicine programs.

Participating in the 15th European Society of Gynecology Conference provided a valuable opportunity for GENTIS and Hanoi Hospital of Andrology and Infertility to connect with the global reproductive medicine community. The presentation showcased the results of years of dedicated research aimed at developing innovative technologies and improving treatment outcomes for infertile couples. This aligns with GENTIS's long-term goal of helping more couples realize their dream of parenthood.

The "dream-like" happiness of a couple both carrying disease gene

Mr. Huy (1983) and Ms. Doanh (1995) in Soc Son, Hanoi had spent about 7 years looking for their children. It can be said that the journey to find their children is more difficult than other couples because Mr. Phu’s legs were bilaterally paralyzed after an accident, and all personal activities almost depend entirely on others. When visiting the hospital, the doctor advised the family to perform a number of tests to find the cause, and found that both Mr. Phu and Ms. Doanh carried the thalassemia gene (congenital hemolysis).

Moreover, 2 miscarriages, 3 failed embryo transfers created a lot of pressure for him and his wife and the treating doctors. Sad, disappointed, but the desire for children and the love that had been wrapped up for 7 years motivated him and his wife to continue to accompany the Vietnamese-Belgian Hospital of Andrology and Infertility (Vietnam -  Belgium IVF).

The happy small family of Mr. Huy and Ms. Doanh

Despite the difficulties, they are still very persistent. Perhaps it was that persistence, perseverance and belief in doctors of the Vietnam - Belgium IVF that created a miracle in the 4th embryo transfer. Baby Minh Dat is the sweet fruit for Mr. Phu and Ms. Doanh's diligent journey to find their children.

However, because economic conditions did not allow it, when doing IVF, they did not perform the preimplantation genetic diagnosis technique. In the case of both husband and wife carrying the thalassemia gene and giving birth naturally, there is a 25% chance that the child will be born with the disease, 50% risk of carrying the disease gene and 25% chance that the healthy child will not carry the disease gene.

With the desire to share, help and support part of the funds to help Mr. Huy's family reduce financial pressure, GENTIS sponsored a thalassemia test to screen Minh Dat early. Fortunately, the results showed that the baby only carried the disease gene.

Receiving the Thalassemia test results, Ms. Doanh choked up emotionally and shared, "My family is grateful and feels very lucky than many people because the baby only carries the disease gene. I wish fathers and mothers who are on the way to find their children will soon pick sweet fruits."

So how is Thalassemia diagnosed?

According to Dr. Ngo Thu Ha (Department of Obstetrics and Infertility of Vietnam Belgium IVF), to diagnose Thalassemia, we should first pay attention to the clinical manifestations and appearance of the patient such as a protruding forehead, flat nose, protruding teeth, enlarged liver, jaundice,... Next, we will do a complete blood count test to check the size of red blood cells. This value is reflected in the mean corpuscular volume index (MCV) and the mean corpuscular hemoglobin (MCH) in your complete blood count test (CBC). If your MCV is <= 85fl and your MCH <=28pg, you are at high risk of carrying a gene mutation that causes thalassemia leading to anemia.

Dr. Ngo Thu Ha (Department of Obstetrics and Infertility of Vietnam Belgium IVF) examines and consults patients

To determine if you have thalassemia, it is necessary to do a hemoglobin electrophoresis test and determine the content of ferritin in the serum, a thalassemia gene test. The implementation of this test will help accurately identify patients with the alpha or beta Thalassemia gene, support doctors in diagnosing and treating the disease effectively, as well as appropriate reproductive advice.
